At Sirona Recovery, we believe that recovery is possible with the right treatment and supports. CCS is one option for individuals seeking help. This voluntary program is available to people who are coping with mental health and/or substance abuse conditions and need help in achieving their personal life goals and visions. Each individual’s experience with CCS is unique as each individual we serve is also unique.
- Milwaukee County resident
- Full Medicaid benefit (may need to confirm T-19 prior to supporting a referral)
- A diagnosed mental health condition and/or substance abuse disorder (alcohol, illegal street drugs, etc.) . If you do not have a formal diagnosis, we will connect you with a provider that can determine a diagnosis prior to supporting a referral to CCS.
- Functional eligibility for CCS will be determined at your first appointment

Comprehensive Community Services (CCS) is built around proven treatment and support methods. All CCS programs are designed to promote and support recovery by stabilizing and addressing an individual’s critical mental health needs, including substance use problems, an individual’s ability to self-manage their physical and social health, and an individual’s ability to meet their basic needs.
Individuals enrolled in CCS work with a dedicated team to decide which programs and supports will assist the individual to reach their recovery goals.
If you are interested in Care Coordination, the first step is to contact CCS Intake at ccsintake@sironarecovery.org. An intake specialist will discuss the CCS program and together, you will discover what needs can be met with our programming. Within 30 days, you will attend a total of four appointments conducted either via phone, video, or face-to-face. If you are already receiving CCS Care Coordination and are interested in Psychological Rehabilitation Services, such as Skills Development, Housing Assistance, Psychosocial Education, and Wellness Groups, please call 414.573.3165 or email ccsancillary@sironarecovery.org.
