Access Point assessments are conducted in the Sirona office located at 205 W. Highland Ave., Suite 400 or the assessment can be arranged at a community site convenient to the client.

We will schedule an assessment that takes up two hours. The in-depth assessment will include questions about mental health history, drug and alcohol use, and other areas of life for you or your loved one. With this information, the Sirona Recovery Access Point staff will connect you to the appropriate programs, level of care, and services that meet your interests and needs.
What to expect
The assessment takes up to two hours.
Please bring a photo ID or driver’s license and your health insurance card (if you have them).
We will guide you through a broad array of questions to identify their needs and best recommendations for treatment and services (history of substance use, mental health, physical health, family/ social supports, employment and education history, involvement in the criminal justice system, etc.)
We respect individual choice and will work collaboratively with you and your loved ones to identify a treatment plan best suited for your needs.
We provide assessments both in person (at the office or other locations to meet client needs) as well as telehealth (phone/Zoom).
Who is eligible?
- Milwaukee County resident
- 18-59 years old
- History of substance use
- You must have an investment in recovery and willingness to actively participate in the program.
